: Verbal consent was obtained from all participants in the NOMS sub-study and documented by the study staff. Ethics committee approval was granted for verbal consent. The study was approved by the Johns Hopkins Bloomberg School of Public Health Institutional Review Board in Baltimore, USA and the Nepal Health Research Council in Kathmandu, Nepal. Health facility data from the SPA and SARA did not include identifiable patient information and ethical approval for data collection was obtained by the original survey implementers.
