: Ethical approval for this study was obtained from the Children’s Hospital of Eastern Ontario Research Ethics Board (13/135X). Under the province of Ontario’s Privacy Act (PHIPA), the Better Outcomes Registry & Network (BORN) as a prescribed registry, and the Canadian Institutes for Health Information (CIHI), as a prescribed entity are allowed to collect, use and disclose personal health information for prescribed purposes, including research, as long as the conditions described under PHIPA are met (PHIPA 2004, c. 3, Sched. A, s. 44 (6)). A Data Sharing Agreement (DSA) is in place between BORN and CIHI.
