Ethics approval and consent to participate

: The study was approved by the Danish Data Protection Agency through the joint notification of The Faculty of Health and Medical Sciences at The University of Copenhagen (record no 2017.08). Further ethical approval or individual consent is not required for register-based research not involving human biological material, according to Danish legislation []. All data analyses were made with anonymized data via remote access to Statistics Denmark and no individual data was visible to the researchers.
