Ethics approval and consent to participate
: A favourable opinion from the London - City & East Research Ethics Committee was obtained in advance of the study (reference 17/LO/0874), alongside relevant management approvals from the Health Research Authority and individual NHS sites. Informed written consent was obtained from all interviewees prior to the interviews, including consent to be audio recorded. Potential participants were informed of the voluntary nature of the study and had at least 24 h to consider taking part.
