Ethics approval and consent to participate

: As a secondary data analysis, this study was exempted from ethical approval by the review board of the Ethiopian Public Health Institute, Addis Ababa, Ethiopia. However, the EmONC assessment protocol was granted a letter of ethical approval by the scientific and ethical review office of EPHI. The EPHI routinely captures anonymous data of reviewed maternal deaths through the MDSR system. Hence, there were no ethical concerns with regard to the MDSR data.We acknowledge the Ethiopian Public Health Institute (EPHI) for allowing us to freely access the database, and no permission and/or a license was required in this regard.
