: Ethical clearance was obtained from The University of Manchester Research Ethics Committee 3 (reference number 2018–4446-6653) in the United Kingdom as well as Catholic University of Allied Health Sciences/Bugando Medical College (CUHAS/BMC) Joint Ethical and Review Committee in Tanzania (reference number CREC/287/2018) and Ethics and Science Converge Institutional Review Board (ERES Converge IRB Zambia; reference number 2018/June 029).
