Ethics approval and consent to participate
: All participants gave informed consent to participate in this study. Written informed consent was obtained, except in cases where the woman was illiterate in which case a witness confirmed the accurate reading of the consent form and a thumbprint of the participant was obtained. This procedure was approved by the ethics committees. Ethical approval was obtained from the WHO Ethical Review Committee, the North East Regional Health Authority Jamaica (NERHA), the University of the West Indies Ethics Committee, the South East Regional Health Authority Jamaica (SERHA), the Kenya Medical Research Institute Scientific and Ethics Review Unit (KEMRI), the Malawi College of Medicine Research and Ethics Committee (COMREC).
