Ethics approval and consent to participate
: This study was granted ethical approval by institutional review boards in all operating counties in addition to the London School of Hygiene & Tropical Medicine (Additional file 14).Voluntary informed written consent was obtained from all observed participants and their families for newborns, who were assured of anonymity and confidentiality. All women were provided with a description of the study procedures in their preferred language at admission, and offered the right to refuse, or withdraw consent at any time during the study. Facility staff were identified before data collection began and no health worker refused to be observed whilst providing care. Voluntary informed written consent was obtained from the respondents for the qualitative interviews who were assured of anonymity and confidentiality.EN-BIRTH is study number 4833, registered at .
