Ethics approval and consent to participate
: The study protocol, consent forms, and survey instruments were submitted to and approved by the Stanford University Institutional Review Board (IRB protocol 43792). Permissions were received from the local Ministry of Health in the study area, and from the directors of all participating hospitals.
