Ethics approval and consent to participate

: This study analyzed data extracted from the NDHS 1996, 2001, 2006, 2011 and 2016 surveys. The ethical clearance for these surveys was obtained from Nepal Research Council and ICF Macro Institutional Review Board in Calverton, Maryland, USA. The DHS data are publicly accessible, and we obtained the permission to use it in May 2019 after DHS reviewed our proposal and we accepted the terms and conditions attached with data sharing policy [CitationRef removed].
