: This was a secondary analysis of data and therefore no further approval was required for this study since the data is secondary and is available in the public domain. However, the source of data (GMHS) reports that Institutional review board of the Ghana Health Service and the Inner City Fund Institutional Review Board (IRB) approved the protocol for the 2017 GMHS.
