: Ethical approval for this study was obtained from the Ethics Review Committee of Faculty of Medicine and Allied Sciences, Rajarata University of Sri Lanka. The first draft was approved under the ERC/2019/07 on 3rd June 2019 and the revised protocol after the pre-test and pilot testing were approved on the 13<sup>th</sup> June 2019. Informed written and verbal consent was obtained from all participants and the legal guardians of pregnant women less than 16 years of age respectively. All methods were performed in accordance with the relevant guidelines and regulations of the Declaration of Helsinki.
