: This study was granted ethics approval by the Papua New Guinea Institute of Medical Research Institutional Review Board on the 7th of August 2014 and allocated the approval number: IRB No. 1414. Ethics approval was also granted by the Papua New Guinea Medical Research Advisory Committee on the 10th of August 2014 and allocated the approval number: MRAC No 14.26. This study adheres to the STROBE guidelines for cross-sectional research (see additional file InternalRef removed). Written consent to collect the data analysed in this study was obtained from provincial and district health authorities, and church health services managing all surveyed health facilities. Prior to data enumeration, oral informed consent was obtained from the officer-in-charge at each of the participating health facilities. All methods were carried out following relevant regulations and guidelines.
