This study was approved by the Institutional Review Ethics Committee (IREC) at MTRH/Moi University (IREC/2014/259), the Review Ethics Board (REB) at the University of Toronto (#31133), and the Institutional Review Board at Indiana University before data collection commenced. A waiver of individual informed consent was granted by the aforementioned Ethics Committees because the research involved minimal risk to the subjects, the waiver was not felt to adversely affect the rights and welfare of the subjects, and the research could not practically be carried out without the waiver (mainly due to large sample size and anticipated significant loss to follow up). Results are reported according to STROBE guidelines for observational research.
