: Ethical approval was obtained from the Ghana Health Service Ethics Review Committee before the commencement of the study; with approval number: GHS-ERC/003/12/22. Permission was sought from the Policy Planning Monitoring and Evaluation Division of the Ghana Health Service. Confidentiality was observed throughout the study.
