: An ethical clearance letter was obtained from Institutional Review Board of Wolaita Sodo University. All the study processes attended to all the basic ethical principles which are respecting their autonomy, and keeping privacy and confidentiality was maintained throughout the data collection process. Operation Room head nurses and delivery room head nurses were informed about the objective of the study and verbal consent was taken and registration books were reviewed. The charts were retrieved from card room after informing the objective of the study to respective bodies and after getting permission.
