Institutional ethical approval was obtained from the Joint Research Ethics Committee of the University of Zimbabwe, Faculty of Medicine and Health Sciences/The Parirenyatwa Group of Hospitals (JREC); reference number; JREC/81/15 and ethics approval at the national level by the Medical Research Council of Zimbabwe (MRCZ) reference number MRCZ/A/1968.
