Ethics approval and consent to participate
: There were two data sources included in this study; both received ethics approval from the Rwanda National Ethics Committee (Rwanda) and Partners Institutional Review Board (Boston, USA). The first data source was a chart extraction of routinely collected clinical data. Because there was no direct contact with patients and minimal risks, both ethics committees waived the need for consent for that aspect of the project. The second data source was collected as part of an ongoing study [CitationRef removed]; women were consented prior to data collection as part of the study. The consent obtained was written.
