Ethics approval and consent to participate

: This study was approved by the NHS REC (13/ES/0129), hospitals participating in Poland (Komisja Bioetyczna Wojskowego Instytutu Medycznego (Military Medical Institute Bioethics Committee, Warsaw) and Komisja Bioetyczna przy Warszawskim Uniwersytecie Medycznym (Medical University of Warsaw Bioethics Committee, Warsaw). From hospital Swietokrzyskie Cancer Centre, we have the approval from the head of gynecological oncology department and University of St Andrews School of Medicine Research Ethics Committee approved the study as a whole (MD10852). Implicit consent was used in the study. Participants were not asked to sign a consent form, however, they were informed that returning the completed questionnaire meant that they agreed to participate in the project.
