: The research protocol for Phase 1 of the study was reviewed and approved by Yorkshire & The Humber Leeds East Research Ethics Committee (REC reference 12/YH/0531). The research protocol for Phase 2 of the study was reviewed and approved by London Surrey Borders Research Ethics Committee (15/LO/2017).
