Ethics approval and consent to participate
: In line with guidance from the NHS Health Research Authority which confirms that formal ethical review is not required for audit and service evaluation [], formal research ethical approval was not requested. All TYA eligible for the study were nevertheless approached with written information about the purpose of the study and written consent was required to access health care records. Study information and request for consent to access health care records was also shared with the parent(s) of 19 patients who were aged 15 years at the time they were approached to participate.
