Ethics approval and consent to participate
: All procedures were in accordance with the ethical standards of the institutional research committee and with the 1964 Helsinki declaration and its later amendments or comparable ethical standards. Local ethics and information governance approval was obtained was obtained from Imperial College Healthcare NHS Trust (Audit Registration number: O&P_007). Patient informed consent was not required given retrospective data collection was anonymised, routinely collected as part of clinical practice and used for purposes of audit.
