: This study did not require consent to participate. Waiver approval from aninstitution/committee was not sought. The NSW Health Records and Information Privacy Act 2002 (HRIPA) recognises that an organisation’s quality improvement activities do not require explicit patient consent additional to that elicited in the original clinical practice interaction.
