: Ethical approval was obtained from the Research Ethics Committee of the Faculty of Medicine, Chiang Mai University (Ref. No. 136/2562) on June 28, 2019, and from the local ethics committees where the trial will be conducted (i.e., The Khon Kaen University Ethics Committee in human research; The Research Ethics Committee, Faculty of Medicine, Prince of Songkla University; and The Research Ethics Committee of Lerdsin Hospital, Department of Medical Services, Ministry of Public Health). Written informed consent will be obtained from all adult patients, while written informed assent and parental permission will be obtained from all adolescent patients and their parents.
