: Clinical samples were collected from Cambridge University Hospitals NHS Trust as part of the PROMPT study, and ethical approval was granted by the local research and ethics committee (LREC number: 02/281 M) and by the multicenter research and ethics committee (MREC number 01/4061). Patients were enrolled into these studies under this ethical framework (PROMPT - ExternalRef removed) which involved obtaining informed written consent.
