: The study was performed in accordance with the Declaration of Helsinki and the need for ethical approval was waived by the medical ethical committee of the Erasmus University Medical Center after review of the study design (MEC-2020–0197). Filing the study protocol for permission in each of the peripheral hospitals resulted in either immediate approval, or approval after filing a separate application with one of the local Ethics Committees (2020.061). Informed consent was obtained from all subjects involved in the study.
