: Ethical approval for the conduct of the 45 and Up Study was provided by the University of New South Wales Human Research Ethics Committee. All participants joined the study providing written informed consent for follow-up through repeated data collection and linkage of their data to population health databases. Ethical approval for this research project was provided by the NSW Population & Health Services Research Ethics Committee (12/CIPHS/31) and the Australian National University Human Research Ethics Committee (2012/504).

: KC is co-principal investigator on an unrelated investigator-initiated trial of cervical screening in Australia (Compass; ACTRN12613001207707 and NCT02328872), which is conducted and funded by the VCS Foundation. The VCS Foundation received equipment and a funding contribution from Roche Molecular Systems, USA. However, neither KC nor her institution on her behalf (Cancer Council NSW) receives direct funding from industry for this trial or any other project. KC is also co-principal investigator on a major implementation program ‘Elimination of Cervical Cancer in the Western Pacific’ which has received support from the Minderoo Foundation and the Frazer Family Foundation, and equipment donations from Cepheid Inc.
