A waiver of consent was granted for the use of the data from the administrative health databases for this study. The Health Research Ethics Board of Alberta grants a waiver of consent provided that the research meets the following criteria: (1) The research involves minimal risk to the participants (2) waiver of consent will not adversely affect the rights and well-being of the participants (3) The research could not be completed without altering the informed consent procedure (4) The proposed alteration is extensively defined and (5) The participants will be given any relevant additional information after participation whenever appropriate.
