The research protocol, interview guide, and all patient communication templates were reviewed and approved by the Western Institutional Review Board (WIRB®), now known as WIRB-Copernicus Group (WCG® IRB; Review number: 44798204). Informed consent was obtained from all participants. Interviews were audio recorded so that verbatim transcripts from each interview could be developed and used as source data for the analysis. All protected health information was removed from the transcripts prior to analysis.
