: Ethical approval was obtained from the Research Ethics Committee of Debre Tabor University (Ref. No. DTU/RP/917/13). Due to the retrospective nature of the data collection from patient medical charts, institutional permission was obtained from each of the selected hospitals after a detail explanation of the purpose of the study to extract patient’s charts and contact the patients through phone call. Individual oral informed consent was obtained by phone from living patients, while consent for deceased patients were obtained from their relatives (husbands, mother father or children over the age of 18 years old). Confidentiality of patient information was maintained. All research methods followed the relevant guidelines and regulations.
