: This study was performed according to the Helsinki declaration. Ethical approval was granted by the Regional Ethics Review board in Linköping (diary number 2018/139 − 31). Due to the retrospective non-interventional design and the fact that the majority of patients were not expected to be alive at time of data collection, the Ethics board waived the requirement for informed consent.
