Ethics approval and consent to participate
: The study has been granted ethical approval by the medical ethical review board at VU University Medical Centre, in Amsterdam, the Netherlands. All interviews were held face-to-face and interviewees were assured that the gathered interview data remained confidential. With the participants’ verbal consent at the start of the interview recorded on tape at the start of the interview. The verbatim interview was checked by all participants for accurateness prior to further analysis. Signed consent to participate in the project and evaluation was provided at hospital level.
