Ethics approval and consent to participate

: This study protocol was approved on 13th April 2015 by the NRES Committee South West – Central Bristol Research Ethics Committee (REC) recognised by the United Kingdom Health Research Authority (HRA) [REC reference: 15/SW/0084]. Permission was also granted by the Confidentiality Advisory Group (CAG) for the team to access selected records for the purposes of extracting anonymised, aggregate-level data (15/CAG/0172).All sites within the UK received Research & Development approval from the appropriate Health Boards and NHS Trusts before commencing study procedures.Consent to participate in interviews, staff shadowing and oberservations was obtained by the study researchers from all participants, following the provision of written information.
