: Written ethical consent has been taken for collection of data from all hospitals through letters number 8075–79/HMC, 488/pharm (KTH), 09 (LRH), and NWGH/Research/02. Ethical principles under the declaration of Helsinki (1964) by the International Conference on Harmonization and Nuremburg Code were followed as Good Clinical Practice (GCP) guidelines [CitationRef removed]. Identification number, medical records and names of the patients were kept confidential. Children was not directly involved in the study therefore consent to participate waived off.
