: Study materials were reviewed and approved by research ethics committees at the Peruvian National Institutes of Child Health (Instituto Nacional de Salud del Niño), Lima, Peru (E-36/14) and Harvard Medical School, Boston, USA (IRB 14–1737). We obtained written informed consent from adult health personnel. For adolescents, we obtained written informed assent from adolescents and written informed consent from a parent or biological family member; in the case a caregiver was not a biological family member, we followed guidelines provided by the ethics committee of INSN to obtain consent from a legal guardian.
