Ethics approval and consent to participate

: All study methods and procedures were approved by the RTI International, University of Minnesota, Docking Institute of Public Affairs, and Fort State University Institutional Review Boards. All participants were over the age of 18. All participants provided informed consent; written consent was provided for the online sample and verbal consent was provided for the telephone and mail samples. The Institutional Review Boards of RTI International, the University of Minnesota, and the Docking Institute reviewed and approved all aspects of the study related to human subjects before and after instrument testing and any revisions.
