: Ethical approval was obtained from the ethical committee of the higher institute of health sciences of the Universite de Montagnes and institute of ethics of health sciences reference number 164/CIERSH/DM/2015. Administrative approval was obtained from all the ten study settings. Written informed consent was obtained from the mother or legal guardian of each neonate before inclusion into this study.
