Ethics approval and consent to participate
: Ethical permission was obtained from Ethical Review Committee of the Institute of Medicine, Tribhuvan University (Kathmandu, Nepal) and the Institutional Review Board of the Johns Hopkins Bloomberg School of Public Health (Baltimore, MD) approved the study. Both Boards provided a waiver of documentation of consent as appropriate for the context. Field staff read the oral consent script to mothers, who provided verbal consent for their infant’s participation; this process was documented for each participant. For all subjects, the study procedures were explained thoroughly as was the voluntary nature of their participation. All of their questions were answered. Confidentiality of the information was maintained by using unique numerical identifiers.
