Ethics approval and consent to participate
: Institutional research ethics approval was obtained from the Hamilton Integrated Research Ethics Board for McMaster Children’s Hospital (#14–214) and the Office of Research Ethics at Children’s Hospital, London Health Sciences Centre. Written informed parental consent and patient assent or consent where appropriate were obtained for participation in this study, and an ethics waiver was obtained from the Office for Protection of Research Subjects at the University of Illinois at Chicago prior to analyzing the de-identified data for this study.
