Ethics approval and consent to participate

: This study was conducted according to the guidelines laid down in the Declaration of Helsinki and all procedures involving human subjects/patients and each site specific protocol was also approved by the ethical review boards of the participating institutions. Participants were considered to have consented once they read the document and signed to the survey. Informed assent was obtained from every adolescent, and all parents and/or legal guardians signed an informed consent. All participants signed a written informed consent/assent before commencing the study. Participants’ confidentiality for the pooled data was maintained using numeric identification codes rather than names. All data transfer was done using a secure file sharing system.
