Participants were considered to have consented once they read the document and signed to the survey. Informed assent was obtained from every adolescent, and all parents and/or legal guardians signed an informed consent. All participants signed a written informed consent/assent before commencing the study. Participants’ confidentiality for the pooled data was maintained using numeric identification codes rather than names. All data transfer was done using a secure file sharing system.
