Ethics approval and consent to participate
: This study was approved by the Medical Ethic Committee of the Leiden University (23252.058.08). All parents and guardians were informed about the purpose of the research with a paper letter, and were explicitly informed that they were free to participate or not without any consequence for the care to be received. This letter was sent to the parents along with a standard invitation for the PCH visit. Given that the study concerned just one moment of data collection per parent, the Committee did not consider separate written consent from parents or guardians to be needed for all participants under the age of 16 years. Therefore written consent was not obtained from any participant nor from any parent/guardian for participants under the age of 16 years.’
