: Ethical approval for this study was waived by the Christian Health Association of Ghana (CHAG) Institutional Ethics Review Board, as the study involved secondary analysis of routinely collected, anonymised clinical data. Administrative permission to access the data was obtained from CHAG and the participating district hospitals.
