Ethics approval and consent to participate

: The study, including the consent forms for participants, was approved by the Regional Ethics Board in Gothenburg, Reference number: 773–13. The consent forms used in this study include information to the participants: that participation is voluntary and (for patients and next of kin) will not affect the care in any way; that the data will be treated anonymously; that data is kept in a safe which only the research group has access to; that it is possible to withdraw consent without explanation; and that the results from the study will be published in scientific journals in a way so that individual persons cannot be identified. The participants read this information and the consent form is signed before data is collected.
