Ethics approval and consent to participate

: We analysed only clinical routine data from health records. Data were anonymized for the analyses. The responsible ethics committee takes the position that such studies do not require approval from an ethics committee (Medical Association of North Rhine [Ärztekammer Nordrhein], Germany, § 15 (1) Professional Code of Conduct of the North Rhine Medical Association [Berufsordnung für nordrheinische Ärztinnen und Ärzte]). The study is a retrospective analysis and not an interventional study. Therefore, it was not registered.
