Ethics approval and consent to participate
: The Regional Committee for Medical and Health Research Ethics of Central Norway approved the study protocols for each follow-up (ref: 78–00 May 2000; 4.2005.2605; 2013/636/REK midt). Participation was based on written informed consent, including written consent from parents or legal guardians at childhood examinations. A protocol for referral was in place.
