Ethics approval and consent to participate
: Information was collected in accordance with health service guidelines, and both Study 1 and Study 2 were reviewed and approved by the University of Wollongong and the Illawarra Shoalhaven Local Health District Health and Medical Human Research Ethics Committee (HE10/371) and site-specific approval was given at the local health district sites. De-identified service use data was provided to researchers in Study 1 thus did not involve explicit written consent procedures. All participants in Study 2 provided written informed consent.
