: The study is conducted according to the principles of the Declaration of Helsinki (version 64th, October 2013) and in accordance with the Medical Research Involving Human Subjects Act (WMO) and other guidelines, regulations and acts. Also, the present study is approved by the medical ethics Committee on Research involving Human Subjects (CCMO). Written informed consent will be obtained from all parents of children participating in the TRACE study ((METC (IRB) approval number 2014–1349) (See Additional fileInternalRef removed). Two protocol modifications were approved by the CCMO. The ethics approval covers both study sites.
