Ethics approval and consent to participate
: The study was conducted in compliance with the Declaration of Helsinki (World Medical Association 2013). As it was part of quality improvement initiatives in the hospitals, an approval from an ethical committee was not necessary. The principles published in the national ethical guidelines for research were followed (SFS 2003:460).As the record review was part of the quality improvement initiatives in the hospitals, consent from the patients was not necessary. Names and personal identification numbers were not collected or entered into the database.
