: This study will be performed according to the principles of the Declaration of Helsinki and in accordance with the Dutch Medical Research Involving Human Subjects Act (WMO) and the General Data Protection Regulation (GDPR). The EINSTEIN study was approved by the Medical Ethics Committee of the Amsterdam UMC, location VUmc (registration number NL74377.029.20). Amendments to the protocol must first be approved by the METC of Amsterdam UMC, location VUmc, before they can be put into practice. The trial is registered with the Netherlands Trial Register (registration number NL8955) it is registered on October 6<sup>th</sup> 2020. All participants in this study provide written informed consent prior to inclusion in the EINSTEIN study.
