: The study was approved by the local data protection officer in the Inland hospital trust (ID 137850). Since the study concerns entirely anonymized data without any person-identifiable information, it was exempted from the requirement of informed consent. The Regional Committee for Medical and Health Research Ethics (REC South-East, University of Oslo) concluded that the project was not covered by the Health Research Act and waived the request for ethical approval of the study.
